Convert all Op plugins from ServiceLoader to op.yaml style #140
Description
We don't want to use ServiceLoader for ops anymore, because it will spoil the completeness of the online op index for example. And any other tool that wants to reason about what ops are available, without loading a JVM or special-casing the service loader mechanism somehow.
